How Registration Works

 

Our programs are not for people who may be tired of their gym routine, would like to try yoga, and could benefit from some psychological tuning

Our programs are for people who find it increasingly difficult to maintain a good quality of life, require assistance with their emotional functioning, and may be at a place in life where they have tried other forms of mental health treatment and are thinking, “Should I try again?” 

 

Eligibility:

We are a clinical program and a professional referral is required.

We provide you downloadable forms to help your professionals fully understand the lifebalance philosophy and approach to mental health treatment before making a referral. This is important so that your timing of group treatment is appropriate and the right group is chosen and matches your needs.

We are a mental health program, not a physical/injury based rehabilitation clinic and this is explained in the downloadable forms. We understand people may have illnesses or injuries that may limit them in the physical realm. We will provide a form for you and your physician so they are fully aware of the lifebalance physical program ingredients. We want your time with us to be healthy and safe.

 

Who can refer?

  • For participation in one of our groups, a referral must be received by a client’s psychologist, psychiatrist, physician, or other medical specialist that is knowledgeable about the client's diagnostic and current status. 

 

  • Professional third-party providers or organizations may refer their clients based on existing current medical knowledge/information. For example, if a disability management organization or representative wishes to refer a client, they must have medical information about the client's diagnostic and current status to maintain an appropriate referral.

 

  • If independent professionals wish to make a referral, (such as a nurse, social worker, occupational therapist, clinical counselor, etc.) they must be someone who has worked closely with the client in a professional capacity for at least 2 months and are aware of the client's diagnostic status provided by the primary medical/mental health provider.

 

All three forms for registration in our programs are available for download and provide further details to help you and your professionals decide if a lifebalance program is appropriate for you.  

All three forms must be completed and submitted prior to being placed on the group list. Payment is never required until the schedule is released and you have enrolled.

Of course treatment is voluntary and we do not accept clients who are simply attending to meet requirements for another program or service, or are being obligated/mandated to attend. This is important to us in helping  to maintain a no-pressure environment that is healthy, supportive, and conducive to change. 


Our registration process occurs in two simple phases.

 

Phase One:  Groups are filled on a first come, first join basis.

  1. Select the group you would like to join.
  2. Complete your on-line pre-registration for the group that you are interested in (you will receive a confirmation email).
  3. Download and complete (PDF's found at bottom of this page) Client Consent, Physician, and Referral forms, and then return to Lifebalance Groups (instructions are provided)

Phase Two:  Once we have your completed forms:

You are placed on the participant waiting list for the group, in priority sequence. Once the 4 - 8 person group quota is reached, a schedule is emailed to everyone. (example below)  

  • Group Therapy: Monday: 5:00 pm (duration - 1.5 hrs)
  • Group Yoga: Tues/Thurs 5:00 pm (duration - 1 hr)
  • Group Fitness: Wed/Fri: 5:00 pm (duration - 1 hr)

If the schedule works for you, you can confirm and then submit your payment in a number of different ways. The payment instructions are the same regardless of whether you or a 3rd party is making payment. 

(Payment is only requested after the schedule is released and you have confirmed enrollment). 

Note* We are aware that finding a schedule that works for everyone can be quite a challenge. If you find the group timings do not fit your schedule this time, you can always stay on the list and you will be notified when the same group is scheduled again, to see if it works for you at that time.

Major Depressive Disorder (MDD)

Description: These groups are for clients having difficulty managing the mental and physical symptoms of depression. These are good groups for clients who experience distress in social, occupational, or other important areas of functioning due to their depression.  

 

MDD Group (open to all)

Posttraumatic Stress Disorder (PTSD)  

Description: These groups are for clients having difficulty managing the mental and physical symptoms of PTSD. These are good groups for clients who experience distress in social, occupational, or other important areas of functioning due to their PTSD. Our current groups are vocational specific groups for First Responders and Military Personnel.

Generalized Anxiety and Panic

Description: These groups are for clients having difficulty managing the mental and physical symptoms of anxiety and panic. This is a good group for clients who experience distress in social, occupational, or other important areas of functioning due to their anxiety.

Adjustment Groups (anxiety and depression)

Description: This group is for clients having difficulty managing the symptoms of anxiety and depression in the context of adjustment to an acquired injury or disability. Often, adjustment to an acquired injury or disability becomes complicated with low mood and anxiety problems.

 

Chronic Pain Management Group * (open to all)

Current Driving Anxiety Group

 

Driving Anxiety Group (open to all)

Description: This is a trauma specific group for those involved in a Motor Vehicle Collision or other related incident where fear of driving has now developed. In some cases moderate to severe generalized anxiety problems can also lead to driving/on-the-road anxiety. This is an excellent group for anyone experiencing an inability to return to their previous level of driving

*Note: In-lieu of the fitness component, this program contains a practical on-the-road component with an instructor who understands patience and pacing. Exposure coupled with therapy and yoga will address this situation specific anxiety/phobia effectively. Our vehicle is equipped with dual steering wheel, dual acceleration/brake, and we can accommodate drivers who require left-foot use due to disability or injury.

Life Skills: Stress & Anger Management for men (gender specific)

Description: This is a Punjabi language group for men focusing on stress and anger management. In addition to a culturally appropriate education delivery, this group provides an opportunity for skills and strategies building for emotional regulation. Management of cognitive and behavioural changes that cause distress in social, occupational, or other important areas of functioning are discussed.

Life Skills: Staying Clean (open to all)

Description: This is a proactive group for anyone that feels their substance-use sobriety may be challenged by recent stressors and difficulty coping

Adult Bullying Trauma (open to all)

Description: This is a group for anyone experiencing difficulties at work or in other areas of life due to bullying that feels inescapable and is impacting mental health

Handling Loss/Bereavement (open to all)

Description: This is a group for clients who have experienced a close personal loss and have developed a persistent bereavement related depression. This is a good group for individuals with significant grief and mourning reactions that are not alleviating or improving within an acceptable time frame and causes distress in social, occupational, or other important areas of functioning.

Divorce and Coping (open to all)

Description: This is a proactive group for anyone challenged by the transition of going through a recent final separation or divorce.

Personal Identity Group (open to all)

Description: This is a group for anyone transitioning through a time in life where personal identity issues are becoming a source of stress – either personally or in terms of the negative responses from others. Examples include revealing that one is gay; making a drastic lifestyle change; leaving an entrenched group or organization; or any situation where one is experiencing difficulty with managing the ‘new self’.

We are unable to predict wait times (we are new and don’t have this data yet).

Our program scheduling is quota based; as soon as the 4 - 8 person quota is reached for the closed group, our program coordinator starts scheduling immediately. Since we only post groups for which a clinician and team is ready, group start dates will be reasonably fast.
